As a Registered Nurse (RN) Case Manager, within our Transition Management Office (TMO), located at Medical Center North on campus serving VUMC, you will collaborate with the health care team to coordinate patient and family interventions across the continuum of care, removing barriers and promoting efficient and effective use of resources. You will have the opportunity to assist patients and families in establishing patient specific goals, both long term and short term; facilitates appropriate follow up care and/or refers patients (within the designated patient population) to the appropriate next level or site of care; serve in an advocacy role on behalf of patients, families, and caregivers to ensure safe, equitable healthcare provision; assist in developing and meeting key Pillar outcomes and system improvement goals including financial, satisfaction, and clinical as the nursing component of the care coordination model.
BSN Graduate of an approved/accredited nursing program; MSN preferred
Two years of RN experience or the equivalent required; prefer 4-7 years
Inpatient hospital experience is required.
Prior experience in case management preferred.
Must be a critical thinker and problem solver, with ability to work independently.
Strong organizational skills, attention to detail, and a positive attitude are vital.
Strong interpersonal and leadership skills in dealing with healthcare providers, patients, and internal staff members.
RN licensure (TN)
