Coordinates referrals between referral sources and CHRISTUS Home Care and Hospice Regions to assure timely, organized and efficient management of patient referrals and admissions, and satisfaction of referral sources with the referral/admission process. Enter prospective and admitted patient demographic information and status updates in the patient database and communication to the regions/branches; Facilitate processing of admission documents from referral call through transfer to appropriate care team; Help resolve referral and admissions management issues and complaints through direct communication with referral sources and admissions team. Responsible for Insurance Verification, Eligibility and Authorizations.
KEY JOB DUTIES / RESPONSIBILITIES:
Obtains initial patient referral information by telephone, fax, email or web portal accounts and completes referrals data entry in computer system on a daily basis. Instructs patients and families on the scope of services. Performs pre-admission screening of referrals to ensure completeness of referral. Obtains a complete history and physical, verifies insurance coverage and coordinates referrals for all CHRISTUS Home Health and Hospice Regions. Weight: 30%
Coordinates patient admission by collaborating with physicians, nurses, case managers, social workers, patients and families at hospitals, assisted livings, and long-term facilities to provide a seamless transition to home care or hospice. Pursues referrals through relationship building with these individuals and their satisfaction with the hospice referral/admission process. Weight: 15%
Manage the referral and admission information to appropriate departments and branches. Weight:15%
Rotate weekend call. Meet 24/7 call and visit scheduling standards. Make follow-up calls for referral inquiries within required time. 7%
