Imperial Beach Community Clinic (IBCC) and the Nestor Community Health Center are Federally Qualified Health Centers (FQHC) that provide community based primary care services to low-income residents of the South Bay area. CRF staff members provide psychosocial assessment, psychiatric services, and ongoing mental health services, co-located within IBCC, to their patients. Services are delivered according to the Behavioral Health Consultant and Integrated Primary Care and Behavioral Health Services model. Respect for the client is a core value, and our staff members work closely with all clients to develop individualized plans that are short-term, solution-focused, and focus on their strengths, needs and abilities. We view the community as holding a wealth of knowledge, experience, and opportunities to help clients move toward improved health and stability. General Description The Licensed Clinical Social Worker provides psychosocial assessments, crisis intervention, follow up appointments, and coordination of mental health services at the IBHC and Nestor sites. S/he is responsible for scheduling psychiatric appointments as needed for psychiatric evaluations and ongoing medication support services. S/he provides mental health services according to the Behavioral Health Consultant model which includes short-term, solution-focused counseling sessions with a maximum duration of 30 minutes for each session, “as needed” consultation to primary care physicians and staff. These services are to be provided to a contracted amount of clients per day as defined by the contract. All direct services will be provided in English or Spanish, based on the client’s preference.
The ideal candidate is a California Licensed Clinical Social Worker. Experience working in a fast-paced work environment and primary care setting is a plus. S/he must also be Bilingual in English/Spanish and must have effective communication skills, have the ability to multi-task, be detail oriented, possess basic computer skills (Excel, Word, and Outlook) and be able to perform autonomously.
