The Director of Enterprise Care Management provides leadership oversight of care management strategies for the operations of the Bronson Network and alignment with Affirmant Health Partners care management (CM) Programs. The Director is responsible to lead, direct and execute standardized processes that are both centralized and imbedded within provider practices that participate with Bronson Network. This includes working with Inpatient and Post Acute Care leadership to ensure Bronson Network's CM Program facilitates smooth transitions for optimal patient outcomes. The Director supports and serves in a consultative capacity to the Bronson Network CI Committee, CM Committee and Affirmant Committee, as appropriate, and senior management to provide analysis and recommendations for improved operational care management and condition management activities.
Current Michigan RN license in good standing required. Bachelor Degree in nursing, business or healthcare administration. Five to seven years' experience in healthcare setting. Five to seven years in a professional and supervisory role. Experience in presenting speeches or formal course work in communications. Demonstrated knowledge of managed care principles. Previous experience in case management, care management and transitions of care.
Master's Degree in Healthcare Management or business related field preferred.
Mental & Visual Fatigue: Work which is constantly and exceptionally tiring mentally and/or visually. Conditions exists more that 90% of the work time.
Physical Effort: The job produces some physical demands. Typical of jobs that include regular walking standing, stooping, bending, sitting, and some lifting of light weight objects.
