Job Description:Â Licensed Outpatient Therapist
The Outpatient Program Therapist performs group, individual, family, and couples psychotherapy to clients who are enrolled in our PHP and IOP programs for mental health, substance abuse, and dual diagnoses. Â This role includes assessment, diagnosis, treatment planning, consultation, psycho-education, crisis intervention, and providing information and referrals as needed. The outpatient program therapist is primarily responsible for maintaining the chart clinical documentation, including treatment plan and treatment plan updates, clinical formulation, biopsychosocial assessment, substance abuse assessment, and discharge summary. Outpatient program therapist acts as the clinical lead in coordinating care among the multi-disciplinary staff involved in patient care.
Valley Hospital is centrally located with easy access to many areas of greater Phoenix. We are between North 35th and 36th Streets with East Earl to our north and East Thomas to our south.
