The Clinical Assessment Center Support Staff Assistant is responsible for ensuring effective communication between the CAC Department and various departmentsâ€™ services within the hospital and the community for the purposes of patient discharge.Â The CAC Support Staff Assistant is responsible discharge flow for all patients deemed inpatient level of care to safely be discharged and transitioned to their community and/or next level of care timely.Â The CAC Support Staff Assistant is responsible for maintaining records in a manner that assures confidentiality of all documents and communications for patient care and services.Â Abide by the Code of Conduct and Corporate Compliance Plan at all times an in all areas.
Knowledge of all disaster, fire, and internal emergency plans and procedures.
Maintains confidentiality procedures for all patients, visitors, and vendors to the hospital.
Demonstrates the security procedures by screening and coordinating with visitors, patients, and family members as they leave the hospital.
Effective oral and written communication skills.
Demonstrates placing a high priority on serving external and internal customers in a timely and courteous manner.Â Follows up until needs are met.Â Demonstrates polite, cooperative and respectful behavior to patients and customers.Â
Demonstrates respect, empathy and consideration toward all external and internal customers.
Ability to prioritize.
Knowledge of the transportation options to and from the hospital; ability to give directions.
Knowledge of the Administrator On Call System.
Recognize and avoid activities and relationships that involve or might appear to involve a conflict of interest or that compromise integrity.
Maintain standards of behavior that are both lawful and ethical and adhere to these standards dealing with patients and their families, with other employees, with vendors, with government regulators or with the general public.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Serves as liason for the units and every patient upon discharge; answering telephones in a professional and timely manner.Â Answers telephone calls by the third ring.Â Collects accurate information, completes necessary paperwork and confirms unit discharge paperwork is complete.
Cordially greets all patients, visitors, and personnel coming to Discharge area.Â Offers assistance and provides direction in a concise, informative, and courteous manner.
Maintains secure and orderly working area.Â Maintains belongings in a safe clean organized way.
Coordinates communication systems in CAC by providing timely clear communication with CAC about the Discharged patient.
May be asked to provide backup coverage for front desk or the units for lunch, and breaks.Â
Accepts and relays messages, and keeps relevant information posted on the bulletin board and in the communication book.
Manages the paperwork of the department.Â Types, copies, files, and distributes information as necessary.Â Audits and confirms that all discharge paperwork is accurate and complete prior to accompanying the patient to their safe transition home or to a community based program.
Confirms checklist of all discharge documentation and instructions have been supplied to the patients and reviews so that the patients clearly understands and signs attesting to such.Â Obtains patient satisfaction survey data.
Maintains needed supplies and equipment in the department.
Maintains a neat, clean, and professional appearance.
Responsible for the reviewing of patient belongings.Â Compares itemized log and obtains signature of patient.Â Secures stored belongings are safely returned to the patient..Â Insures valuables logged in the safe are returned.Â Obtains patient signatures.
Keeps accurate log of any transportation vouchers supplied to patients.Â Notes what where why and when.
Conducts tasks, assignments, and interactions in a manner that indicates physical and emotional capability of performing position responsibilities.Â Demonstrates ability to relate well to patients, visitors, family members of patients and staff.
Dependable and punctual in attendance.Â Position is 8:30 am â€" 5:00 pm Monday â€" Friday yet may fluctuate based on the needs of the patient flow for PIW.Â Can be asked to work weekends or different hours.
Attends Flash daily to obtain confirmation of discharges.
Completes assigned work in a dependable and timely manner.
Demonstrates a professional attitude and supports the objectives of the hospitalâ€™s marketing and guest relations philosophy through internal and external communications and interactions with all levels of staff, patients, family members, guests, community, and referral sources.
Participates in all activities as requested or assigned by Senior Management staff.
Initiates follow up calls to external referral sources.Â Tracks data.
Responsible for entering DC data into the MS4 system.
