Job Description The Division of Prevention and Primary Care (PPC) works to advance population health through supporting access to high quality health services and by introducing innovative community and system changes that promote disease prevention and control in New York City. The Division builds upon a history of cutting-edge policy and programming in primary health care delivery systems and communities and applies an integrated public health and clinical care approach in its work to improve population health, prevent chronic disease, and advance equitable health outcomes. The Health Access Equity Unit, housed in PPC, seeks to develop and implement clinical and community models and interventions for improving health outcomes for people with a history of involvement in the criminal justice system, grounded in primary care. The unit leverages strategic research and health surveillance, provider education and training, clinical and social systems programming, and policy analysis to advance approaches that address health and social needs of people with a history of justice involvement living in communities. The Health Access Equity Unit's signature initiative, The NYC Health Justice Network, unites primary care and social service organizations in an innovative system of care delivery and navigation for people with a history of justice involvement and their families. The NYC Health Justice Network approach includes trauma and resilience-informed care and engagement with community health workers. Under the leadership of the Executive Director, the unit and its staff will drive evidence-based and evidence-informed approaches to improve the health of people with a history of justice involvement and to transform systems - especially the primary care delivery system - to address the unique needs of the population and their families. The Executive Director will be responsible for oversight of the unit's activities, including strategic guidance for the direction of the unit, supervision of staff, and engagement with internal and external partners. DUTIES WILL INCLUDE BUT NOT BE LIMITED TO: - Develop and promote a strategic vision for the Division's approach to addressing mass incarceration as a public health issue with special attention to solutions anchored in primary care. - Direct all aspects of the Unit's work, including program and strategy development and oversight and management of clinical and technical research and programming. - Oversee implementation and evaluation of the NYC Health Justice Network, a multi-year initiative seeking to improve health outcomes for people with justice involvement through engagement with primary care and community partners. - Drive clinical and public health research to inform the Unit's work and to contribute to the local and national conversation on justice involvement and health. - Oversee development, implementation, and evaluation of training of providers, community members, and others on topics relevant care for people with a history of justice-involvement, with a specific focus on trauma and resilience-informed care. - Liaise and collaborate with other PPC bureaus and DOHMH divisions to optimize resources and expertise and engage with external partners including other city agencies, task forces, community organizations and advocates to inform priorities. - Lead, inspire and mentor staff to accomplish the goals of the organization. - Oversee the Unit's administrative operations, including staff supervision and mentoring, hiring of new staff, and management of funds. - Identify and act on opportunities for strategic engagement with key partners, including health care and others, to transform systems of care delivery, employment, and navigation for people with a history of justice involvement. - Chair clinical and community advisory groups that guide development and implementation of the Unit's work. - Monitor and inform relevant legislation, rules and other local, state and national policy. - Participate in Divisional activities, including leadership meetings, strategic planning, and reporting. - Foster development of new initiatives and secure funding for implementation. - Prepare manuscripts, memos, and reports, as well as give presentations on the Unit's work.
