The Addiction Coordinator, as part of an interdisciplinary team, including the patient/family, physicians, nurses, clinical pharmacist, therapists and payers ensures that patients referred to the Geisinger’s addiction medicine services receives coordinated care for medical, behavioral and addiction needs.
The incumbent interacts with patients, family members, healthcare professionals, and community and state agencies in this effort. The Coordinator serves as a liaison between the hospital and community agencies or facilities for the exchange of clinical and referral information.
The Coordinator identifies high risk patients from a psychosocial/financial perspective, assesses the psychological needs of patients and families and provides information, support, counseling, care management, and referrals to appropriate resources.
The Coordinator works collaboratively with the addiction care team to ensure patient needs are met and care delivery is coordinated across the continuum. The expertise of the Coordinator is sought to resolve psychosocial patient care issues and to develop and implement a complex patient plan of care, in collaboration with the primary care physician, addiction specialist, clinical pharmacist and other specialty care team members. This professional ensures that patients are motivated to engage with chronic (Medication Assisted Therapy) management of their substance abuse or dependency needs, in coordination with other behavioral or medical health care needs.
The incumbent performs duties and tasks in accordance with performance standards established for the job. The incumbent is responsible for participation in and completion of all patient safety initiatives appropriate to the position. In addition, the incumbent conducts all job responsibilities according to the Mission and Values of the organization.
Responsible for ensuring the very best patient/family experience by optimizing all customer interaction. Foster a positive work environment through mutual support and cooperation and adheres to GHS and unit specific Service Standards.
The incumbent reports to the Director of Care Management.

EDUCATION AND/OR EXPERIENCE:
Bachelor’s Degree in Social Work (BSW), Psychology, or related field of study from accredited university required.
Care Management certification is required after two years of hire.
A minimum of two (2) years of clinical experience as Coordinator, Case Manager or Social Worker in addiction or substance abuse services required.
Experience in dealing with substance abuse, behavioral health and other community healthcare agencies.
A working knowledge of insurance coverage, various governmental programs, and available community care agencies that support the social determinant of health
Experience with motivational interviewing preferred.
WORKING CONDITIONS/PHYSICAL DEMANDS:
Work is performed in a clinical office. Some travel to patient’s homes, or other levels of care may be required.
