PurposeStatement / Position Summary

Performall Nursing positions within the Inpatient and Specialty Support Departmentworking independently in any shift, any day of the week and holidays.  Positions may include; Hospital Based AdmitTeam Nurse, Telephone Advise Nurse, Medication Refill Nurse, Inpatient CaseManager, Discharge Planner and act as a liaison between Healthplan, Hospitalstaff and Primary Care Providers.

Essential Functions and Responsibilities of the Job

1.Ability to operate Microsoft operating systems.

2.Apply evidence-based criteria (i.e.: Milliman, Interqual); and/or Health planmedical policy.

3.Experience in Case Management and Discharge Planning.

4.Experience using electronic medical record or electronic health recordpreferred.

5.Comply with Per Diem Availability agreement.

6.  Be at work and be on time.

7.  Followcompany policies, procedures and directives.

8.  Interactin a positive and constructive manner.

9.  Prioritizeand multitask.

10. Adhere to Per Diem Memorandum.

Telecommuting (full or part time) is amanagement option and not an employee entitlement. The organization willconsider a request to telecommute on a case-by-case basis depending on manyfactors, including but not limited to business necessity and availability ofresources.
