The Lead Transition RN is responsible for assisting the nurse manager with their day to day tasks. The individual in this role will perform such duties as assuming responsibility of the Transition Services department in the absence of the nurse manager, delegating nursing assignments, training and educating staff. In addition to these tasks, the lead nurse also may carry a patient load depending on department staffing.
