The Patient Care Assistant II is an unlicensedcaregiver working under the direction of the Registered Nurse.  This position is responsible for supportingthe Neurosciences and Orthopedics Unit (Acute Care Service) to maintain smooth,effective communication between healthcare practitioners, departments,patients, and visitors.   The PatientCare Assistant II position is accountable for the accurate, timelydocumentation in the patient medical record. Ensures prompt, timely assistance which contributes to the patient'sprogress towards discharge.  This role isaccountable for patient admission, transfer, discharge flow processes, and forproviding the most basic of patient care. including data collection, personal hygiene, vital sign measurement andassistance with activities of daily living.
