Position Summary:

The Duke University Hospital (DUH) Ventricular Assist Device (VAD) Nurse Practitioner (NP) / Coordinator collaborates with patients and the multidisciplinary team to formulate and carry out a plan of care for patients being evaluated for or post ventricular assist device implant in the inpatient and outpatient settings in collaboration with the Heart Failure medicine and cardiothoracic surgery teams. The VAD NP performs physical exams, collects, documents, and analyzes clinical data, conducts, and interprets diagnostic and therapeutic procedures, orders and schedules laboratory studies and professional consultations, prescribes appropriate interventions and medications, coordinates the care and discharge of patients, provides direct patient care services, and provides education to staff, patients, significant others, and the community. The NP contributes to excellence in patient care in the inpatient and outpatient clinic settings, research, teaching, and provides leadership to the organization.

Work Hours

• Full-time salaried with usual hours dayshift (9-6) Monday through Friday. Variations in hours and days of week covered may occur to meet patient and service needs, for example, to cover operative procedures or patient emergencies or to perform weekend care or discharges.

• On-call coverage is required after hours, on weekends, and on holidays to be rotated and shared among the VAD team clinical members. The schedule and hours of coverage may be changed by Heart Center leadership in consultation with clinical service line leadership based on service needs.

Responsibilities:

• The VAD NP conducts rounds and assesses patients with VADS, including performing history and physical exams, writing notes and orders, performs pre-VAD insertion evaluations, and collaborates with establishing plan of care with multidisciplinary team.

• The VAD NP plans and arranges transitions of care across the health care spectrum and orders, monitors, and interprets inpatient and outpatient tests and diagnostic procedure results to improve patient outcomes and prevent complications and readmissions.

• The VAD NP will care for patients in the inpatient and outpatient settings with responsibilities including office visits, outpatient laboratory monitoring and medication titration, follow-up phone calls, and triage.

• The VAD NP provides expert consultation on multiple VAD systems, including basic care, parameters, and troubleshooting, for patients and staff at DUH and in collaborating care practices during normal working hours and while covering on call responsibilities.

• The NP provides training to patients (pre- and post-implant and in evaluation phases), caregivers, students, residents, and other clinicians on VAD systems, including alarm management, troubleshooting, and management of VAD-related issues.

• Additional responsibilities of the VAD NP include collaboration with the INTERMACS database and research coordinators to ensure timely screening, consent, and monitoring of clinical trial patient outcomes. Additionally, the VAD NP will be an active participant in program quality improvement initiatives and research projects to improve outcomes using evidence-based care.

Knowledge, Skills & Abilities:

Education

• This position requires completion of an accredited Nurse Practitioner (NP) program, with national certification as an NP from a national certifying board. The NP must demonstrate and maintain credentialing and licensing requirements of Duke University Hospital and The North Carolina Medical Board and/or North Carolina Nursing Board. The NP must have AHA healthcare provider status for BLS and ACLS certification.

Experience

• The VAD NP candidate should have two years of nursing experience , caring for VAD/cardiac assist device patients is strongly preferred.

• Other clinical experience that is desirable and preferred includes past employment in a cardiac area such as cardiothoracic or coronary care intensive care unit, a cardiac step-down unit or progressive care unit, the cardiac catheterization or electrophysiology lab, a heart failure unit, or a clinical research unit.

Work requires completion of an accredited Nurse Practitioner program, with aMaster's degree in Nursing

No experience required. 3 years of RN experience strongly preferred.

Must have current or compact RN licensure in the state of North Carolina. BLS required. National certification as a NP required prior to or within 6 months of employment.
