The incumbent provides social work services and case management services for veterans in the Home Based Primary Care (HBPC) program within the Geriatrics and Extended Care Line at the Chalmers P. Wylie VA Ambulatory Care Center. Veterans in the HBPC program are homebound and have complex medical and psychosocial needs. The incumbent makes home visits to veterans in HBPC program in order to provide social work services.
