How would you like to become a part of a team providing compassionate care to Veterans? This position is stationed at the Milo C. Huempfner VA Health Care Center (HCC) in Green Bay, Wisconsin, and organizationally located within the Primary Care Division's Whole Health Program of Clement J. Zablocki VAMC (ZVAMC), Milwaukee, WI. For this Whole Health position, in addition to the Social Worker, we are recruiting from the following disciplines: Registered Nurse, Registered Dietician, and Occupational Therapist. The Whole Health Program is a comprehensive team approach which delivers care to Veteran patients in a longitudinal rather than episodic fashion and which focuses on prevention; health promotion, coordination and chronic disease management. The social worker in this Veteran-centric approach often functions as a care coordinator to ensure that health care meets the needs, as defined by the Veteran. Social workers are prepared for this role by virtue of their education, training, and experience which prepares them to understand the person-in-situation and the influences that family and community have in impacting the health care needs of Veterans. Contacts eligible participants, actively and passively, for enrollment and effectively explain the Whole Health process. Provides certified health coaching for Veterans in order to reduce or eliminate high-risk behaviors while increasing health ones. Conducts interventions for Veterans using one-on-one or group modalities in outpatient and inpatient settings. Provides education and experiential opportunities to engage Veterans in complimentary and integrative approaches, including but not limited to, guided imagery, meditation, mindful awareness, etc. Conducts Whole Health Facilitated Groups; participate in MOVE, smoking cessation, diabetes management, hypertension clinics, mental health, and other inpatient and outpatient groups as appropriate. Provides follow-up care through telephone clinics, VA video connect or secure messaging between face-to-face appointments. Actively guides Veterans in the exploration of core elements of personal health planning through individual and group-based programs. Encourages and assists Veterans with the adoption of habits that are conducive to a higher quality of life and in alignment with the Veteran's goals.
