The Physician Assistant has specialized skills and training, who works collaboratively with members of the Vascular Surgery Section staff. His / Her areas of responsibility encompass: Vascular Surgery Clinics, including Pre-Operative and Post-Operative Evaluations, Minor Procedure Clinic, as well as areas outside those sites as required for care, consultation and collaboration with patients, their families, nursing staff, physicians, multidisciplinary staff, and other healthcare team members Duties include but are not limited to: Evaluating, treating, and educating patients with various cancers. Provides effective care to patients characterized by the use of sound judgment in assessing, planning, implementing, documenting, and evaluating patient care for assigned patients. Obtain thorough medical histories, perform comprehensive physical examinations, order appropriate diagnostic studies, interpret and analyze medical data, establish diagnoses, formulate and implement treatment plans, and modify treatment plans when appropriate. Assist with prescribing medications and treatment regimens within their scope of knowledge and as indicated for treatment of the above. Perform initial evaluations and physical examinations on new and follow-up oncology patients. Provide ongoing outpatient care for assigned patients. Screen outpatients to determine need for further health care. Order diagnostic studies such as laboratory tests, x-rays, and other studies necessary for the care of patients. Review diagnostic test results and implement appropriate treatment. Provide education and counseling of patients and families in the care of diagnosed cancer, and use of prescribed treatments and medications. Assist the Supervising Physician (or agent) in evaluation and management of patients. Carry out health promotion and disease prevention activities. Request non-VA care cancer consultation. Concurrence by a physician is required in before consultation may be approved. Assist other departments by providing information necessary for data collection. Perform other duties as assigned, including telemedicine activities. CLINICAL: 1. Incumbent works collaboratively with the Vascular Surgery staff and resident in both outpatient and telehealth clinics, and participates in daily rounds of the Vascular Surgery inpatients. 2. Ensure urgent/ emergent patient care issues are addressed quickly with attending surgeon/fellow/chief resident and arrange admissions/ transfers as needed. 3. Work with case manager for special admissions: bridging anticoagulation, prehydration for CTAs, angiograms, etc., and ensure H&P and orders are placed. 4. He/she assists with managing wounds by sharp debridement with instruments, dressing, solutions, suture removal, incision and drainage. 5. Under the direction of a vascular surgeon, the candidate performs: a. Consult review b. Prior record review c. Patient history, examination, diagnosis, treatment and education for new and follow up visits d. Responds to Clinical Reminders e. Orders appropriate lab tests, radiology tests, and studies by Provider Order Entry f. Interprets as appropriate and the implementation of results & recommendations from studies ordered. g. Initiates and adjusts medications as medically necessary h. Recommends appropriate follow up i. Performs routine wound care for post-surgical patients with vascular wounds. j. Completes electronic encounter for each visit k. Conducts telephone clinics and responds to patient phone calls as appropriate I. Respects time commitments m. Provides customer service appropriate to VA standards n. Obtains outside medical records needed for patient care o. Consents and counsels patients regarding surgeries/interventions. ADMINISTRATIVE 6. Utilizes Advanced Clinical Access (ACA) principles 7. Assists in meeting Performance Measures for Vascular Surgery 8. Coordinates care for patients as appropriate between vascular clinic, VISN, community, and primary care 9. Incumbent schedules outpatient clinic appointments using Veterans Health Information Systems and Technology Architecture (VistA) scheduling options. Follows nationally established business rules to schedule appointments for veterans. This includes clinic visits, consultations and entries into the Electronic Wait List (EWL). Staff member completes the mandatory training requirements for the outpatient scheduling process. 10. Prescreen clinic lists the week before to ensure each patient has necessary studies prior to appointment. 11. Ensure timely patient encounters and complete documentation 12. Attend Wednesday conferences and follow-up on tasks assigned in conference. 13. Follow-up patient care communications by email/ telephone or MyHealtheVet. 14. Attend Vascular section division meetings. GENERAL 15. Participates in Peer Review 16. Works as a team with vascular surgeons, nursing, technicians, primary care, administration, lab, pharmacy, dieticians, and ancillary staff on behalf of patient care 17. Completes CME, LMS, HIPAA, Security training, and other training as mandated by VA 18. Serves on committees, teams, workgroups, and chart reviews as requested/approved by administration 19. Meets standards of Physician Assistant licensing and certification 20.
