This is an open continuous announcement; applicants will be reviewed and referred on a bi-weekly basis; the announcement will remain open until the position is filled. This vacancy will be used to fill Licensed Practical Nurse positions within various areas of Ambulatory Care (Athens/Aiken CBOC, Medicine Clinic & Patient Aligned Care Team-PACT) Financial Disclosure Report: Not required Ambulatory Care - Medicine Clinic Work Schedule: Monday - Friday 8:00am-4:30pm Duties: - Work as part of RN/LPN care team - Respond to secure messages - Perform vital signs and record - Competence with EKG, taking blood for glucometer, obtain lab specimen, diabetic foot screening, screening of patients as needed, simple dressing change - Assist Providers with procedures - Conduct non-complex phone calls backs/follow-ups Preferred Experience: 1-2 years full time clinical experience as a LPN in Medical/ Surgical setting, ED, Urgent Care or Primary Care preferred. Specialized experience is experience that has equipped you with the particular competencies to perform successfully the duties of the position, and that is typically in or related to the work of the position to be filled. Qualifying specialized experience for this position (which should be documented on your resume). Athens/Aiken Duties: Ambulatory Care - Athens/Aiken Community Based Outpatient Clinic (CBOC) Work Schedule: 8:00am-4:30pm Assist Primary Care Provider with procedures Competence with EKG, taking blood for glucometer, obtain lab specimen, diabetic foot screening, administration of vaccinations, providing patient education regarding tobacco use, weight monitoring, screening of patients as needed, simple dressing change. May be assigned to cross cover in other clinical areas within his/her competency level. Stocking of rooms, ordering of supplies, maintains and uses oxygen equipment Conduct non-complex phone call-backs/ follow-ups Perform vital signs and record Conduct clinical reminders and equipment checks Preferred Experience: 1-2 years full time clinical experience as a LPN in Medical/ Surgical setting, ED, Urgent Care or Primary Care preferred An understanding of clinical reminders and performance measures are preferred; Computer literate (knowledge of CPRS/ Outlook; Excel spreadsheets a must) ; Flexible, customer service friendly and have the ability to multi task; He/ she must be able to adapt quickly to change beyond the traditional role assignment and work with administrative and interdisciplinary team members. PACT Duties: Ambulatory Care - PACT (Patient Aligned Care Team) Work Schedule: Monday - Friday 8:00am-5:00pm, some extended weeks of 4-nine hour days and a 4 hour Saturday Proficient in performing EKG, glucometer use, collecting lab specimens, diabetic foot screening, administration of vaccinations. Provide patient education regarding ambulatory sensitive conditions. Provide patient education regarding ambulatory sensitive conditions, screening of patients as needed, simple dressing changes. Demonstrate evidence-based behaviors that positively influence patient outcomes and team experience. Proficient use of Microsoft programs; Outlook, Word, and Excel. Preferred Experience: A minimum of 5 years Primary Care or Direct Acute Care experience.
