The Nurse Clinical Case Managers work closely with the VA Puget Sound Patient Advocate Team to provide clinical assessment, nursing diagnosis, plan development and implementation/coordination of complex medical, mental health and psychosocial needs of Veterans during the time of an active complaint/concern being addressed within the department. This is followed by a follow-up evaluation and closure of process once goals are met. Duties include but are not limited to: Develop measurable and achievable short term and long term goals for the client based on the assessment and diagnosis previously made with the agreement of the Veteran Utilize existing and future clinical tools including but not limited to: Mission Act and VA Community Care guidelines; CPRS and consult tool box functions including screening/triage tool; Triwest Healthcare Alliance and secondary authorization review process; additional VA and Department of Defense (DoD) EMHR including Joint Legacy Viewer and MHS Genesis, Cerner, Virtu Pro, HealthShare Referral Manager Partner with VA Healthcare System, VA Community Care and DoD system healthcare partners to address the Veteran issue during this process Pay: Competitive salary, regular salary increases. Paid Time Off: Annual leave at the rate of eight (8) hours per biweekly pay period; Sick leave at the rate of four (4) hours per biweekly pay period; 10 paid Federal holidays per year. Retirement: Traditional federal pension (5 years' vesting) and federal 401K with up to 5% in contributions by VA. Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement). Licensure: 1 full and unrestricted license from any US State or territory.
