The Behavioral Sleep Nursing Coordinator functions as the liaison and case manager for Sleep Behavioral Services for VA Puget Sound. Coordination of care includes closing gaps in communication and patient handling, increasing inter-department communication, improving patient access to provider clinics, improving integration of community care and increasing patient/staff satisfaction. The Behavioral Sleep Nurse Coordinator works directly with behavioral sleep psychologists, providers, fellows and residents, technicians and administrative staff to ensure timely care management of patients with sleep disorders. The Nurse Coordinator promotes use of cognitive behavioral therapy for insomnia instead of sleep medication avoiding adverse events. The Nurse Coordinator case manages patients with the goal of improving adherence to positive airway pressure (PAP) therapy through behavioral programs. Based on education and experience, may be assigned responsibilities as preceptor or other teaching opportunities. The RN will cross-train and cover the current Sleep Medicine Nurse Coordinator. Responsibilities and Duties include but are not limited to: Coordinates team meetings to review complex patient care plans; and improve patient flow and access to care Leads quality improvement projects based on Whole Health Guidelines and Recommendations Serves as a resource person for patients, families, and staff Participates in research and educational activities Coordinates patient care and places associated orders Manages MyHealthyVet secure messaging Tracks patient care needs utilizing Care Management Tool Software Performs telephone triage and routes patients appropriately Performs PAP data download and contacts Veterans to identify barriers to PAP usage Work Schedule: Monday-Friday 8:00am to 4:30pm Financial Disclosure Report: Not required Recruitment/Relocation Incentives: Not Authorized Permanent Change of Station (PCS): Not Authorized Pay: Competitive salary, regular salary increases. Paid Time Off: Annual leave at the rate of eight (8) hours per biweekly pay period; Sick leave at the rate of four (4) hours per biweekly pay period; 10 paid Federal holidays per year. Retirement: Traditional federal pension (5 years' vesting) and federal 401K with up to 5% in contributions by VA. Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement). Licensure: 1 full and unrestricted license from any US State or territory.
