The CLC RN serves a population of male and female Veterans from the adult to the elderly adult, age 65 and over. The RN practices in Geriatrics, Extended Care and Rehabilitation within Patient Care Services and specializes in Gerontology, Geriatric Rehab, and Dementia. The geriatric patient population is likely to present with chronic medical conditions, change in mental status, and/or functional decline. The RN is accountable when assigned as a primary care or team leader for a defined group of patients for the entire episode of care. The RN is accountable for defined groups of patients on his/her shift. Based on education and experience, may be assigned responsibilities as a preceptor, charge nurse, or unit shift leader. Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. The Nursing, Geriatrics, Extended Care, and Rehabilitation Service, Community Living Center, (GEC - CLC) RN works directly with managers. The RN, GEC-CLC is accountable to the Associate Chief Nurse (ACN), Nurse Manager (NM) and Assistant Nurse Manager (ANM). The RN also works with other licensed medical professionals to ensure adequate care is delivered to all patients. The RN is accountable when assigned as a primary care or team leader for a defined group of patients for the entire episode of care.
