The Eastern Colorado Veterans Health System (ECHCS), Mental Health Service Line is currently recruiting for one (1) Social Worker in the Life Skills Center/Psychosocial Rehabilitation and Recovery Center (PRRC) at the Rocky Mountain Regional VAMC, Colorado. This position requires the person to have a valid and unrestricted state issued drivers license and must be submitted with your application.. Duties Assists Veterans to self-determine their goals and to select from a wide range of services to develop skills, support and wellness strategies needed to achieve the goals for an improved quality of life. Will be expected to remain informed of best practices within the fields of mental health and psychosocial rehabilitation and recovery. Responsible for providing clinical care for Veterans living with serious and persistent mental illness enrolled in PRRC which also includes clinical responsibility for the ongoing practice of assessment, goal setting, treatment planning and transition/discharge planning. Will be responsible for facilitating daily educational, recovery-oriented and curriculum-based classes and psychotherapy groups in the PRRC. Will provide services to Veterans in the community as it fits with developing skills for enhanced community integration. Will be responsible for educating Veterans eligible for PRRC services through Bridge groups in both the inpatient and outpatient mental health settings. Provides comprehensive social work services to Veterans with serious mental illness and ensures the veteran's care is appropriately coordinated with the VA Medical Facility, community health or mental health providers, and agencies that serve Veterans including assisted living facilities. Professional social worker and functions as a clinical social worker for the VA Eastern Colorado Health Care System's PRRC program.
