This Social Worker - Transition and Care Management (TCM) position is located in the Social Work Service of the Tomah, Wisconsin VA Medical Center. The Social Worker assesses and treats the complicated psychosocial problems of Veterans. The incumbent is a professional Social Worker whose duties and responsibilities relate to the care management of severely ill, polytrauma and/or injured Operation Iraqi Freedom (OIF), Operation Enduring Freedom (OEF), Operation New Dawn (OND) service members, medically retiring/transferring from Medical Treatment Facilities (MTF), and Veterans. The incumbent serves as the social worker on the Polytrauma Team. The Social Worker at the full performance level of the GS-11. Duties include: The Social Worker at the full performance level of the GS-11. Duties include: -Independently complete thorough psychosocial assessments to determine the psychosocial functioning and needs of Veterans and/or their families. -Actively participate as a member of the interdisciplinary treatment team through collaboration with Veterans, caregivers, family as well as interdisciplinary treatment team members in the development and implementation of treatment goals and interventions. -Possess a working knowledge and experience in use of medical and mental health diagnoses, disabilities, and treatment procedures, including acute, chronic and traumatic illnesses, common medications and their effects/side effects, and medical terminology. -Possess knowledge and ability to independently implement treatment modalities, provide psycho-educational classes, and/or supportive groups for Veterans and families. -Provide consultation to other treatment team and staff members regarding psychosocial needs of Veterans and/or their families and the impact of the identified psychosocial problems on the Veteran's health care planning and compliance with treatment. -Possess skills and knowledge to provide case management services to Veterans and their support networks throughout a wide continuum of care to include VA and community based resources. -Possess and demonstrate ability to communicate effectively and professionally, both orally and in writing, with co-workers and community stakeholders. -Will participate in a rotation of on-call social workers, including modification of tour of duty to accommodate patient care needs on weekends if indicated.
