| The Geriatric RN Case Manager uses the nursing process and evidence-based practice to collaborate with the Patient Aligned Care Team (Patient, Gerofit clinic Provider, Primary care Provider, Clinical Support Technician, family/caregiver, internal and community based services involved in providing care) in developing the patient-driven, holistic care plan for life. Pay: Competitive salary, regular salary increases. Paid Time Off: (Part Time) Annual leave accrued at the rate of one (1) hour per every 10 hours in a paid status; Sick leave accrued at the rate of one (1) hours per every 20 hours in a paid status. Retirement: Traditional federal pension (5 years' vesting) and federal 401K with up to 5% in contributions by VA. Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement). Licensure: 1 full and unrestricted license from any US State or territory. Incentive(s): EDRP/PCS/Recruitment/Relocation incentive is not authorized for this position Gerofit is an exercise and wellness program for Veterans 65 and up which offers both in-person and virtual (VA Video Connect) exercise classes. We have exercise sessions three days a week which are designed to help the Veteran improve their cardiovascular endurance, upper body strength, and balance. The Gerofit RN will be involved in all aspects of the program, both clinical and administrative, as part of an interdisciplinary team. Duties include but are not limited to: As part of a collaborative team approach assists the Gerofit program director and program manager in all operations of this program. Triages and applies a collaborative team approach in identifying and analyzing patient care problems; documents the nursing process to address the patient care problems with respect to the team. Conducts scheduled and unscheduled in-person, telephone, VVC, and VCM Gerofit appointments addressing Veteran participation in the program; Complete patient screenings and intake assessments whether in-person, or via VVC, VCM, or telephone. Provide direct nursing care as indicated Provides education and promotes health promotion and risk reduction strategies Assists with navigation of health care systems and intervenes or refers as needed to facilitate the achievement of patient outcomes. Serves as a coordinator within the organization regarding Gerofit outpatient care Work Schedule: Monday to Friday, 7:30am to 12:00pm clinic hours Financial Disclosure Report: Not required
