This position is located in the Geriatrics & Extended Care, Community Living Center, Salisbury VA Medical Center, Salisbury, NC. The Nursing Assistant also performs assigned duties in a manner that promotes quality health care, outstanding customer service, and the maintenance of an environment committed to serving the Veteran. The incumbent patient/resident assignments typically involve more complex nursing needs which can vary within range of predictable to unpredictable requirements. The incumbent must be able to satisfactorily carry out any duty of GS-5 Nursing Assistant(s), and satisfactorily perform the following nursing procedures. Complete basic technical nursing procedures (in accordance with classroom instruction and correct clinical technical) for providing and monitoring direct Resident care typical of the assigned unit. Also, participates in therapeutic individual and group activities including all aspects of cultural transformation; observes Residents for changes in mental and/or physical status; and demonstrates correct interventions to restore or maintain Resident welfare and safety. Duties include but are not limited to the following: This position will provide continuous observation and surveillance of assigned patients. The first line of action will be to verbally redirect the patient from engaging in any at risk behaviors. Next will be to summon the nursing staff if the patient requires assistance. Monitor staff will be knowledgeable and abide by patient and institutional confidentiality and related Medical Center policies. Demonstrates understanding and proficiency of the use of the Telesitter software. Maintain visual observation at all times. Verbally redirects patient over digital 2-way audio device that is in patient room. Immediately summons the nursing staff if the patient requires assistance. Demonstrate the knowledge and skill necessary to provide the appropriate observation to the patient population requiring Telesitter monitoring. Participate in hand off report with on-coming monitor staff when arriving for shift, and upon completion of shift. Reviews pertinent information and behaviors. Provides Veteran with explanations as necessary but does not counsel or provide opinions. Provides floor staff with information in a timely fashion when Veteran appears to pose a threat to themselves. Participates in a collaborative identification and reporting of patient safety issues. Seeks assistance with removal of any potential safety hazard from room and reports findings to appropriate staff members. Demonstrates safe work practice and attitude, follows safety rules, works to prevent unsafe conditions and behaviors, and participates in organizational and department safety programs. Maintains patient dignity and privacy. And All other duties as assigned by Service Line Leadership. Work Schedule: Various work schedules, with a rotating shift as needed, to include weekends and holidays. Functional Statement Title: Nursing Assistant (NA) GS 5.
