The St. Cloud VA Health Care System is currently recruiting for a few, Full-Time, Permanent, LPNs. These positions are located in the Rehabilitation & Extended Care Service Line. Hours worked is based upon available positions. Applications will be reviewed on a weekly basis until the closing date of this open continuous announcement, as long as vacancies exist. Duties include, but are not limited to: 1. Provides nursing care to patients according to established facility procedures adhering to all VA and nursing policies, procedures, standards and protocol. Nursing care includes but is not limited to: a) Personal Hygiene - bathing, oral hygiene, denture care, shaving and grooming. b) Positioning and Activity-positions veterans using principles of body mechanics and proper lifting and positioning device. c) Nutrition and Fluids -assists veterans at meal time following safety factors, uses adaptive equipment as indicated, manages tube feedings and care of tube per facility policy, measures and records intake and output as indicated. d) Elimination - assist veterans with bowel and bladder elimination as indicated, collects specimens as ordered, measures and records output as indicated, manages urinary drainage devices as directed per facility policy, performs· bladder scans per facility policy. e) Respiratory- positions veterans to promote breathing and maintain open airway, assists with set up and administration of oxygen therapy as ordered, performs oral and naso-pharyngeal suctioning as ordered. f) Skin - observe, maintain and promote skin integrity by following evidence based practice guidelines, facility policies, procedures and protocols. g) Support-interacts with veteran's family members in a kind and caring manner especially during end-of-life, assists with care of body after death. h) Data Collection -obtains records and reports vital signs and measurements which include temperature, pulse, respirations, blood pressure, height, weight, peripheral blood sugar, pulse oximetry and pain. i) Communication -Reports significant changes or concerns in the patient's physical or psychosocial condition to the RN/Provider. Appropriately screens and intervenes in situations requiring a report to the RN/Provider. Work Schedule: The tour of duty will be a mixture of day, evening, and/or night shifts with rotating weekends and holidays. Additional information will be provided during the interview process.
