The Case Manager is a Nurse with strong clinical knowledge and leadership skills. The Case Manager collaborates with the patient, family, physician, interdisciplinary team, and payer representative to assess, plan, implement, coordinate, monitor and evaluate options and services to promote optimal patient outcomes. The Case Manager monitors the provision of cost-effective care to ensure care is given in the most clinically appropriate setting. The Case Manager facilitates compliance with regulatory requirements and organizational performance standards.

Education

Minimum:Graduate of an accredited school Nursing, Associate's Degree in Science, or other qualification recognized by the State of Florida for RN licensure.

Preferred: Bachelor of Science in Nursing (BSN)

Experience

Required: Two (2) years acute hospital patient care experience.
Preferred: Acute hospital case management experience.

Certifications

Required: Florida RN license; BLS certification issued by the American Heart Association

Preferred: Accredited Case Manager (ACM)
