This is full-time permanent position. The incumbent is assigned to supervise a Grant and Per-Diem team at the Brooklyn campus; travel to other campuses and to a transitional housing site in the community is part of position responsibility. The candidate will be responsible for supervising and providing comprehensive case management for multiple domains, including; mental health, SUD, homelessness, medical, benefits eligibility, and other concrete needs. The assignment carries requirements for providing supervision, administrative oversight, attending administrative meetings and case conferences, sitting in on the Homeless Screening Committee and interagency and VISN meetings and calls, providing quality assurance, holding the community partner accountable for the terms of their grant proposal and the directives of the program, managing billing, inspections of the program, overseeing admissions and other duties as required. This includes ensuring staffs' accurate and timely reporting and documentation, i.e., psychosocial assessments, treatment planning, progress notes and HOMES. Familiarity with the Grant and Per-Diem program, Critical Time Intervention, Housing First is essential. A driver's license is also required. Social Work Coordinator must be able to develop appropriate professional relationships with community stakeholders. The Social Worker Grant and Per-Diem Social Worker/Coordinator is responsible for ensuring safe emergency housing, quality rehabilitation, monitoring and coordinating services provided by community-based contracted facility under a grant with Grant and Per-Diem services for homeless Veterans. Social Worker is aware of community resources and provides information on housing, benefits and other local resources. The Grant and Per-Diem Coordinator has experience that demonstrates possession of advanced practice skills and judgment. At least two years should be in an area of specialized social work practice. The advanced practice social worker can be further differentiated from the independent practice social worker by the incumbent's ability to expand clinical knowledge in the profession, provide consultation and guidance to colleagues, role-model effective social work practice skills. Continuity of care also includes coordination with the local Suicide Prevention Coordinator(s) (SPC) to enhance suicide prevention amongst homeless Veterans encountered through community-based programs. Primary duties include, but may not be limited to: Supervise a small team of other Grant and Per-Diem Coordinators. Monitor the clinical care at a transitional housing site and ensure that emergency transitional housing facilities are compliant with the deliverables specified in the Grant and Per-Diem grant by the community agency. Facilitate and compile facility site inspections utilizing the VA Grant and Per-Diem regulations and directives. Expected to coordinate the four inspectors and Contractor to ensure its completion Provide case management to Veterans, as needed, throughout the continuum of care and outreach services to homeless Veterans in communities and rural areas distant from the medical Center which includes but is not limited to participate in case conferences, complete psychosocial assessments, provide crisis intervention, screening/referring veterans for HUD-VASH section 8 benefits, and assist veterans in navigation through the VAMC health care system as well as any needed external community resources as needed Perform duties independently; reporting to Homeless Programs Manager or designee relative to the professional practice of social work, time & attendance, and formal performance evaluation Provide the customer with consistent information according to established policies and procedures, Handles conflict and problems in dealing with the consumer constructively and appropriately Participate in daily VISN 2 downstate Homeless Screening Committee conference calls, and monthly NYH VA Homeless Coordinator's meeting to discuss program activity and other calls as needed Track program activity (admissions/discharges/placement) on veteran's CPRS medical record, VA Homeless Operations Management and Evaluation System (HOMES) data base, and Department of Homeless Services (DHS) online tracker on an a continuous basis Enter/request 2237s on VISTA, review and approve monthly billing and track overall GPD program budget Facilitate quarterly GPD meetings with Community Vendor Discuss with community vendor any pending matter in need of resolution, receive and monitor incident reports/warning notices, receive and approve extension request if clinically appropriate, and review and determine approval overnight passes Have knowledge and skill in the use of word-processing, Excel, VISTA, data management and other computer systems, especially those programs in use by VHA.
