This position is located at VA Western New York Healthcare System (VAWNYHS). The PRRC Social Worker is full time clinical position stationed at Community Day Center which services Veterans across the continuum of care. - Provides services to patients diagnosed with Serious Mental Illness, and families who are experiencing a variety of psychiatric, medical and social problems utilizing individual, group skills. - Participates in the assessment of psychosocial functioning and needs of patients and their family members in order to implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills and their recovery goals. - Participates in admission/screening process by interviewing patients and/or stakeholders and completing intake data collection with purpose of establishing treatment for Severely Mentally Ill (SMI) psychiatric disorders in a PRRC setting. When applicable, includes obtaining psychosocial history, maintaining data sheets, completing high risk screens, answering consults, and developing discharge planning assessments. - Delivers variety of treatment modalities in working with individuals, families and groups to achieve recovery goals. - Participates in or facilitates selected groups relates to the treatment of specific SMI psychiatric disorders, conducts individual counseling and other social work privileged functions independently or under supervision as professional license dictates. - When applicable conduct psycho educational activities following professionally prepared resources guides - Functions as a primary Recovery Coordinator for Veterans on a prescribed caseload of SMI Veterans. This includes preparation and implementation of treatment plans developed in concert with the incumbent's multiciliary team. Considers medical and mental health diagnosis, disabilities, treatment procedures, and medications to formulate a treatment plan. - Acts as a liaison between staff, Veteran and stakeholders in the development and review of treatment plans. - Participates in periodic review of ongoing treatment of patients under the care of the PRRC team. - Provides ongoing documentation of patient's progress using a behavioral framework - Provides counseling and or psychotherapy services to individuals, groups and families using the knowledge of mental health diagnosis, disabilities and treatment procedures. This includes acute, chronic and traumatic illness, common medications and their side-effects and medical terminology. - Maintains knowledge of community resources, making referrals to community and other agencies for services and coordinating those services - Act as clinical consultant on social work practice to other staff and incorporate social work practice into patient treatment through the program. - Acts a resource and liaison person to public and private providers and agencies to assure continuity of care and The incumbent will promote Recovery Oriented Care Principles, Evidence Based Practice for Social Work and patient centered care practice with Social Work Staff.
