The John J. Pershing Medical Center is seeking two full time Licensed Clinical Social Workers to join the Mental Health Care Line, in Poplar Bluff, Missouri. The Substance Abuse Treatment Program (SATP) social worker (LCSW) assumes responsibility for the provision of substance abuse outpatient services to veterans. The SATP social worker interviews patients and significant others and completes psychosocial assessments. The assessment will include data pertinent to the development of an effective treatment plan as stipulated in JCAHO's standards. The social worker will serve as a member of the multidisciplinary treatment team. In addition, the social worker will provide direct care and treatment where indicated and as such, function as a treatment team member. The social worker must have a generic understanding of psychodynamics and use a wide range of treatment modalities, including crisis intervention, individual case work, group work, patient education, confrontation, supportive therapy, discharge planning, knowledge of aftercare treatment resources, etc. The treatment modality shall be consistent with the needs, as well as, goals and objectives of each case. Individuals shall be involved with and participate in formulating and implementing their own rehabilitation efforts insofar as they are capable. The social worker providing substance abuse services may participate in conducting orientation sessions for new employees. In service training sessions may also be conducted for medical center staff to improve the level of knowledge and understanding of substance abuse, its impact on families and significant others, treatment and aftercare services. The social worker obtains a thorough working knowledge of aftercare rehabilitation community treatment resources and makes appropriate referrals. The incumbent makes every effort to link patients to the appropriate resource and to implement the aftercare plan. The social worker serves as a liaison between inpatient setting and the community services. The incumbent advises significant others about community rehabilitation resources available to them (Al-Anon) as well as informing aftercare facilities about VA programs and resources. The social worker is accountable for proper clinical file recording and case work services rendered. The incumbent is responsible for completing the following duties/responsibilities: The incumbent will conduct psychosocial assessments to determine needs of veterans and family members. The incumbent is an active participant in the treatment planning process with other disciplines. The incumbent will participate in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate. The incumbent will provide individual and group counseling as appropriate. The incumbent will provide consultation/education to veterans and families on community resources, advance directives and VA benefits. The incumbent will act as a consultant to other team/staff members. The incumbent will act as liaison between VA and community resources, to include marketing and public relation duties as needed. The incumbent will provide case management to veterans throughout the continuum of care. The incumbent will provide a full range of social work services within commonly accepted standards of social work practice which includes case work, individual and group counseling, discharge planning, case management and other services as appropriate. The incumbent will address Advance Directives and Organ Donation in accordance with the standards of their work area. The incumbent will provide wellness/prevention education. The incumbent will as appropriate, supervise the practice of social work students, or a less experienced social worker. The incumbent will coordinate treatment and communicate with the care lines to facilitate continuity of care for veterans. The incumbent will participate in committee assignments, as deemed necessary by the service manager. The incumbent will maintain data and statistical compilations to comply with JCAHO, VA and medical center policy and procedures. The incumbent will assist veterans and their significant others with coping and dealing with the loss and grief experiences in disability, terminal illness and death. The incumbent will document social work interventions and activities in veteran's clinical record utilizing CPRS and ensure appropriate hand off when transferring of patient care. The incumbent will participate in a rotation of on-call social workers, including modification of tour of duty to accommodate patient care needs on weekends.
