The RN-GYN Case Manager/Maternity Care Coordinator functions as a nurse case manager and care coordinator by serving as a liaison between the patient, the non-VA provider, and the VA facility; ensuring a seamless transition between facilities and all levels of care. Assumes responsibility for the coordination of care for the Women's Health specialty population. This includes the transition through the continuum of care, follow-up care and supporting factors that impact quality patient care.
