The Registered Nurse (RN) CC&ICM Program Coordinator will partner with the CC&ICM Social Worker to implement the national CC&ICM framework which serves a patient population with highly complex health and mental health problems to ensure they receive the right level of care coordination support based on their prominent need. The incumbent implements, coordinates, and manages all administrative and clinical aspects of the national CC&ICM framework. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
