Orlando Health Physician Associates is one of the largest multi-specialty healthcare groups in Central Florida with over 150 practicing physicians.
Incoming physician should be ABIM Certified (previous endocrinology experience or fellowship preferred) and will develop and advance a comprehensive diabetes management program (DMP), addressing the ever growing, community wide need to target uncontrolled diabetic patients, pre-diabetic patients, and provide more focused support and resources to established members with diabetes. The primary care diabetes management program will serve as a health system resource and will maintain close clinical relationships with endocrinology, cardiology, nutrition, bariatric surgery and more.
Specific Program Aims:
Development of a multifaceted, primary care-based diabetes management program that addresses obesity and related chronic illness through management of nutrition, glucose monitoring, pharmacotherapy, diabetes education, as well as exercise and behavioral modifications.
Development of quality standards and clinical protocols following established HEDIS quality measures to evaluate the program outcomes, led by Diabetologist.
Implementation of specific quality improvement initiatives in response to program variation from goal while consistently improving the programs structure and process.
Comprehensive Patient Education, Diabetes Self-Management Education and Support (DSMES), in the various aspects of diabetes management.
