Registered Nurse - Care Manager
The RN Care Manager is an essential component of the PACT Initiative (Patient Aligned Care Teams). The RN Care Manager provides nursing assessment, telephone triage with advice per protocol, patient education and case management responsibilities throughout the health care continuum. Collaboration and networking with multiple disciplines, services and agencies is a necessary component of this role. Basic Requirements: English Language Proficiency.
