SummaryThe mission of HBPC is to provide comprehensive - interdisciplinary - primary care in the homes of Veterans with complex medical - social - and behavioral conditions for whom routine clinic based care is not effective. The HBPC target population is frail and/or chronically ill Veterans who require interdisciplinary health care teams - continuity and coordination of care - and the integration of diverse services to cover their complex medical - social - rehabilitative - and behavioral care needs.QualificationsBasic Requirements:English Language ProficiencyIn accordance with 38 U.S.C. 7403(f) - no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English
