The position is for a Registered Nurse to work with Primary Care Women's Health (WH) Substance Abuse Provider to manage the medical and behavioral health needs for a panel of women veterans. Duties include, but aren't limited to: Applies the nursing process and evidence-based practice to collaborate with Interdisciplinary Team members and others to coordinate daily activities of the WH Substance Abuse Patient Aligned Care Team (PACT) Proficient in assessment and triage of women's physical and mental health issues and enacts appropriate interventions and follow-up. .Responsible for building relationships and collaborating with services, internal and external to the VA, to facilitate care.
