OverviewThe Case Manager, Nurse is responsible for providing patient case management services to assigned populations. Utilizes a collaborative interdisciplinary approach to Emergency Department follow-up, outpatient and inpatient needs while anticipating, integrating, and coordinating the patient's plan of care, discharge plan, and re-entry into the community. The goal of the intervention is to meet the needs of patients in order to optimize clinical and resource outcomes, and to monitor the effectiveness of the plan and its outcomes.
